后腹腔镜肾盂成形术治疗成人肾盂输尿管连接部梗阻的临床评价

The Clinical Value of Retroperitoneal Laparoscopic Dismembered Pyeloplasty for Ureteropelvic Junction Obstruction in Adults

目的评估后腹腔镜肾盂成形术治疗成人肾盂输尿管连接部梗阻(UPJO)的临床价值.方法收集从2004年6月至2008年3月19例(21侧)施行后腹腔镜离断肾盂成形术UPJO患者的临床资料,对手术技巧、临床效果及并发症进行分析.结果21侧肾盂成形术无中转开放手术,手术时间为82～166 min,平均(113±45)min;术中出血25～86 mL,平均(37±14)mL.术后6月复查B超和IVU,肾积水均明显减轻或消失.结论后腹腔镜肾盂成形术安全、创伤小、疗效好,值得临床推广.

Objective To evaluate the clinical value of retroperitoneal laparoseopic dismembered pyeloplasty for ureteropelvic junction obstruction in adults. Methods From June 2004 to March 2008, the clinical data of 19 cases （21 sides） of patients with UPJO underwent retroperitoneal laparoscopic dismembered pyeloplasty were collected and analyzed retrospectively. Results Retroperitoneal laparoscopic dismembered pyeloplasty was performed successfully in all 19 cases. The operative time ranged from 82 to 166 min [mean （113 ± 45 ） mini. Bleeding volume ranged from 25 to 86 mL [mean （37 ± 14） mL] and no blood transfusion needed. Hydronephrosis was obviously alleviated and renal function was improved after 6 months. Conclusion Retroperitoneal laparoscopic pyeloplasty is safe, effective and less traumatic for UPJO.
