超声引导下微创经皮肾镜气压弹道治疗肾结石98例报告 被引量:4

摘要 目的探讨超声引导下微创经皮肾镜气压弹道碎石(MPCNL)治疗肾结石的疗效和安全性。方法回顾性分析2005年10月-2007年11月采用MPCNL治疗98例肾结石患者的临床资料,其中鹿角形结石42例、肾脏单发结石27例、多发结石29例。结果98例患者均成功建立皮肤肾脏通道,79例行Ⅰ期碎石(80.7%),19例行Ⅱ期碎石(19.3%);手术时间和结石清除率鹿角形结石组分别为(97±21)min和59.5%(25/42),肾脏单发结石组为(71±13)min和77.8%(22/27),多发结石组为(101±18)min和72.4%(21/29)。无严重并发症发生。结论超声引导下MPCNL治疗肾结石安全、疗效可靠。 Objective To investigate the efficacy and safety of the treatment of nephrolithiasis by minimally invasive percutaneous nephrolithotomy (MPCNL) with lithotripsy under ultrasound guidance. Methods The clinical data of 98 cases with renal calculi underwent MPCNL during Oct. 2005 to Nov. 2007 was retrospectively reviewed. Of the cases, 42 had staghorn calculi, 27 had simple calculi, and 29 had multiple calculi. Results The percutaneous renal access was successfully established under ultrasound guidance in all patients. 68 cases were treated with one-stage operation (69.4%), 19 with two- stage operation (19.3%). The operative time and stone-free rate were (97±21)min and 59.5% in staghorn calculi, (71±13)min and 77.8% in simple calculi, and (101± 18)min and 72.4% in multiple calculi. No severe complication was noted during nephrolithotripsy. Conclusion The treatment of nephrolithiasis using MPCNL appears to be effective and safe under ultrasound guidance.
