脉搏波速度,心-踝血管指数和踝臂指数评价血管早期病变 被引量:21

The clinical value of pulse wave velocity(PWV),cardio-ankle vascular index(CAVI) and ankle brachial index(ABI) in evaluation of early vascular diseases

摘要 目的:探讨脉搏波速度(Pulse Wave Velocity,PWV),心-踝血管指数(Cardio-Ankle Vascular Index,CAVI)和踝臂指数(AnkleBrachial Index,ABI)在男性健康人群和动脉粥样硬化性疾病及其危险因素人群中血管早期病变评价的应用及各指标间的相关性。方法:56例男性受检者入选此研究,平均年龄(57.8±9.8)岁(范围41-78岁),其中23例为健康志愿者(健康组),36例存在动脉粥样硬化性疾病及其危险因素(动脉粥样硬化组)。应用脉搏波速度自动测量系统和血压脉搏测量装置测定颈动脉-股动脉PWV(CF-PWV)、CAVI和ABI作为反映受检者血管早期病变的指标。结果:与健康对照组相比,动脉粥样硬化组CF-PWV和CAVI明显升高分别为(12.61±2.99)m/s比(10.87±1.40)m/s(P=0.005);7.34±0.78比8.63±1.57(P〈0.001),ABI在两组间无显著差异(1.08±0.12比1.09±0.13,P=NS)。相关分析显示,动脉粥样硬化组CAVI与CF-PWV显著相关(R=0.612,P〈0.001),ABI与CF-PWV显著相关(R=-0.454,P=0.012),ABI与CAVI显著相关(R=-0.697,P〈0.001)。健康组上述各指标之间均无明显相关性。结论:动脉粥样硬化及其危险因素人群中反映动脉僵硬度的指标CAVI与PWV密切相关,并与下肢动脉阻塞程度相关。 Objective:To explore clinical value of pulse wave velocity(PWV),cardio-ankle vascular index(CAVI) and ankle brachial index(ABI) in evaluation of early vascular disease in male healthy subjects and atherosclerosis risk patients.Methods:56 male subjects included in our study,mean age 57.8±9.8years(range41~78years),23 healthy subjects(healthy group),36 atherosclerosis risk patients(atherosclerosis group).Automatic PWV measurement system,blood pressure and pulse measurement device were used to measure carotid-femoral PWV(CF-PWV),CAVI and ABI as parameters to reflect early vascular disease.Results: In comparison to healthy group,CF-PWV and CAVI were higher in atherosclerosis group(12.61±2.99 vs 10.87±1.40m/s,P=0.005;7.34±0.78比8.63±1.57,P〈0.001;respectively),ABI had no significant difference between two groups(1.08±0.12比1.09±0.13,P=NS).Relationship analysis results demonstrated that in atherosclerosis group CAVI closely related to CF-PWV(R=0.612,P〈0.001),ABI closely related to CF-PWV(R=-0.454,P=0.012),ABI closely related to CAVI(R=-0.697,P〈0.001).All these parameters had no significantly relationship in healthy group.Conclusions: CAVI and PWV had close relationship in atherosclerosis patients,could reflect arterial stiffness,CAVI and PWV were closely related to the extent of lower extremity obstruction.
