瘢痕性秃发的皮瓣修复

Repair of Cicatricial Alopecia by Skin Flap Graft

【目的】探讨瘢痕性秃发的皮瓣修复方法。【方法】1998年2月至2008年7月收治由外伤、烧伤、溃疡、疖肿等所致的瘢痕性秃发患者37例，其中2例行分次切除缝合，4例行秃发区切除＋局部头皮瓣转移修复，31例行头皮扩张术修复。修复瘢痕性秃发最大面积22．3cm×25．8cm。【结果】除1例扩张头皮瓣部分坏死，行二期头皮扩张，扩张头皮瓣转移修复。其余皮瓣均完全成活，术后随访2年，效果满意。【结论】头皮扩张，扩张头皮瓣转移是修复瘢痕性秃发的一种有效方法。

[Objective] To discuss different procedures of repair in cicatricial alopecia by skin flap graft. [Methods] From 1998 to 2008, different methods were applied to repair 37 cases with cicatricial alopeeia after traumata, burns, ulcer and furuncle and carbuncle or other occasion. Two cases had been repaired by excision and suturing, and 4 cases had been repaired by excision and local scalp flap transplantation and 31 cases had been repaired by expanded scalp flaps graft. The maximal cicatricial alopecia size was 22. 3 cm ×25. 8 cm. [Results]Thirty six eases obtained complete success with satisfactory results. One case had the necrosis of the expanded scalp flap and delayed wound healing. [Conclusion] The expanded scalp flap graft is an effective and convenient method to repair cieatricial alopecia.
