非洛地平对高血压患者红细胞膜结合钙的影响

摘要 目的比较非洛地平缓释片和氢氯噻嗪对高血压患者治疗前、后红细胞膜结合钙的影响。方法按中国医科院阜外医院Fever研究规划,入选70例高血压患者,年龄50~70岁,血压160~180/90~100mmHg,经3个月安慰剂观察,采用多中心双盲分为非洛地平缓释片2.5~5.0mg/d治疗组(T组)和氢氯噻嗪12.5-25mg/d对照组(C组)各33例。经5年随访,对比两组治疗前后血压、心率、心电图、空腹血糖、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL—C)、肌酐、尿酸、血清钾和红细胞膜结合钙。结果T组33例治疗前收缩压和舒张压分别为(159.65±9.51)mmHg和(95.96±3.07)mmHg,治疗后降到(134.09±8.85)mmHg及(82.78±4.21)mmHg,P〈0.01;红细胞膜结合钙治疗前为(2.24±0.3)μg/mg,治疗后上升到(3.21±0.27)μg/mg,P〈0.01;总胆固醇、甘油三酯均有下降。C组有2例脑卒中(均为脑出血),T组未发现心脑血管事件。结论非洛地平缓释片和氢氯噻嗪均有良好降压作用,治疗组脑卒中死亡少于对照组。非洛地平缓释片对红细胞膜结合钙有升高作用,同时对总胆固醇、甘油三酯有降低作用。 Objective Prospective study to compare Felodipine and Hydrochlorthiazide treatment of hypertension with red blood cell membrane calcium inspect. Methods According to chinese medical research institute Fuwai Hospital Fever research plan, selected 70 hyportcnsion patients, Age 50-70 year old, Blood pressure in 160-180/90-100 mm Hg,after 3 month placebo observation. Divide by the multi-centers and double blindly into Feilodipine 2.5-5.0 mg/d treatment group (T group) and Hydrochlorthiazide 12.5-25 mg/d control group (C group) each 33 examples. Respectively five years, contrasts two groups treated before and after, Measured Blood pressure, Heart rate, Electrocardiogram ( EC G), and fasting blood glucose ( FBG ), Total Cholesterol (TC), Triglycerlde (TG), High density lipoprotein ( HDL-C ), Serum creatiniine (Scr), Uricaeid, bloed serum potassium and make red blood ceB membrane calcium determination. Results Before treatment T group of 33 example the systolic pressure and diastolic pressure (159.65±9.51)mm Hg and (94.961±3.07)mm Hg, after treatment falls to (134.09±8.85) mm Hg and (82.78±4.21)mm Hg,P〈0.01 ;The Red Blood cell membrane calcium before treatment for (2.24±0.3) μg/mg, after the treatment rises to (3.21±0.37)μg/mg, P〈0.01. Total Cholesterol and Triglyceride drop too. C group has 2 cerebrovascular events (for cerebral hemorrhage), T group has not discovered cardiocerebrovascular events. Conclusion Felodipine and Hydrochlorthiazide all efficacy decread blood pressure but plendil also can decread TC,TG,red blood cell membrane calcium and decread cerebrovascular events events predominant than Hydrochlorthiazide.
