近视伴共同性外斜视戴矫正镜后斜视角大于矫正前的相关因素研究

摘要 目的研究双眼近视伴共同性外斜视患者戴矫正镜后的斜视角大于戴矫正镜前的相关因素,探讨其可能存在的原因。方法回顾性分析15例双眼近视伴共同性外斜视戴矫正镜后斜视角大于矫正前的患者资料,总结其视力状态、屈光状态、调节与集合功能以及比较矫正前与戴矫正镜状态下使用三棱镜加遮盖法检查视远及视近的斜视角。结果15例患者矫正视力正常,屈光状态呈现高度近视及近视性屈光参差,调节超前,斜视角检查:视近时屈光矫正后与矫正前斜视角平均相差为(2.7±2.1)△,差异无统计学意义,视远时屈光矫正后的斜视角大于矫正前的斜视角,平均相差为(11.8±4.3)△,差异具有显著统计学意义(P<0.01)。结论双眼近视伴共同性外斜视患者视力矫正后的斜视角可大于未矫正情况下的斜视角,此种现象可能与模糊像性调节有关。 Objective To study the correlation factor for that squint angle of myopia with concomitant exotropia with redressing mirror more than that without minus lens, and to explore the possible reasons. Methods Retrospective analysis was taken on data of 15 patients, whose squint angle of myopia with concomitant exotropia was more than that before wearing redressing mirror. Vision, refraction, accommodation and convergence were summarized, and squint anqle was compared between before and after wearing redressing mirror under prism and alternative covering method when viewing closely or distantly, respectively. Results All fifteen patients had normal vision, high myopia and myopic anisometropia under refraction,and excessive accommodation to anterior part. With examination of squint angle,there was no difference of squint angle after correction and before correction when they viewed closely, the average difference of squint angle was(2.7 ±2. 1 )^△ ,but obviously difference was found when they viewed distantly,the average difference of squint angle was( 11.8 ± 4.3 )^△ (P 〈 0.01 ). Comelusion Squint angle of myopia with concomitant exotropia after correction could be more than that before correction and it may connect with accommodation to image blur in myopia.
