摘要
目的探讨多项心电图电压标准在高血压左室构型中的表现。方法研究Cornell、Sokolow-Lyon、Gubner-Ungerleider、∑QRS指数与高血压左室构型相关性,并比较四种电压标准在异常左室构型的敏感性、特异性。结果Sokolow-Lyon指数在向心性重构组,向心性肥厚组增高;②∑QRS指数:异常左室构型组均增高;③Cornell、Gubner-Ungerleider指数仅在向心性肥厚组增高。另一方面:向心性重构组:Sokolow-Lyon指数、∑QRS指数增高;②向心性肥厚组:四种电压标准均增高;③离心性肥厚组:仅∑QRS指数增高。结论∑QRS指数对高血压异常左室构型有着高度敏感性,而Cornell与Gubner-Ungerleider指数有着高度的特异性。四种电压标准在不同的左室构型有着相应的改变。
Objective To explore the association of the left ventrieular geometry and multiple electrocardiogram criteria in essential hypertensive patients. Methods Echocardiography and electrocardiogram was used in 85 essential hypertensive patients in order to evaluate the sensitivity and specificity of four ECG criteria in diagnose of abnormal left ventrieular geometry. Results Firstly, Sokolow - Lyon index was higher in the concentric remodeling and concentric hypertrophy groups than in the other group. Secondly, ∑ QRS index was higher in abnormal geometry in comparison with those in normal ventrieular geometry group. Finally, Comell and Cubner - Ungerleider index was significantly higher only in the coneemrie hypertrophy groups. On the other hand ,The concentric remodeling group showed increased voltages for Sokolow - Lyon and ∑QRS index.. The four electrocardiogram criteria were found to be higher in the concentric hypertrophy group. Only the ∑ QRS index show significantly higher than the other electrocardiogram criteria in the eccentric hypertrophy group. Conclusion The ∑ QRS index showed higher sensitivity , while the Comell and Gubner - Ungerleider Index showed higher specificity in the diagnosis of abnormal geometry. Different patterns in the left ventricle have their own characteristics of electrocardiogram criteria.
