摘要
目的研究体外循环心脏不停跳手术与停跳手术对心脏肾素-血管紧张素-醛固酮系统(RAAS)影响的差异。方法将40只健康山羊分为实验组(浅低温不停跳组)与对照组(中度低温停跳组)进行体外循环,分别于7个时点采冠状静脉窦血,采用放射免疫分析法检测肾素(PRA)、血管紧张素Ⅱ(AⅡ)和醛固酮(Aldo)水平。结果两组在体外循环过程中冠状静脉窦血PRA、AⅡ和Aldo均有不同程度的升高(P<0.01),其中对照组比实验组升高更明显(P<0.01)。结论体外循环心脏不停跳心内手术对心脏RAAS的影响可能较停跳心内手术小。
Objective To study the differences of the effect on coronary sinus renin-anglotensin-aldosterone system (RAAS) between beating heart surgery and cardioplegia surgery during cardiopulmonary bypass (CPB). Methods Forty goats were randomly divided into experimental and control group, experimental group ( n = 20) ,operated on beating heart surgery with mild hypothermic CPB, control group (n =20) ,operated on arrested heart with moderate hypothermic CPB. Coronary sinus plasma renin activity (PRA) ,angiotensin Ⅱ (All) and aldosterone (Aldo) of two groups were measured by radioimmunoaasay at seven different time intervals, respectively. Results Coronary sinus PRA,A Ⅱ and Aldo in two groups increased in varying degree during CPB. PRA,A Ⅱ and Aldo in control group increased more highly than those of experimental group. Conclusion The results imply that the effect on coronary sinus RAAS in beating heart surgery during CPB is less than that in cardioplegia surgery.
