肾脏结核的CT影像特点

CT features of renal tuberculosis

目的探讨肾脏结核的CT影像特点。方法回顾分析经病理证实或临床证实20例肾结核患者的临床资料,男9例,女11例,18例临床表现尿频、尿急、尿痛、血尿,2例临床症状隐匿,所有患者均行螺旋CT肾脏平扫,增强的动脉期、静脉期和延迟期的全尿路成像,分析CT表现特点。结果肾脏结核CT主要表现为:肾积水(80%),静脉期花瓣样强化(70%),肾周脂肪模糊(60%),肾盂、输尿管管壁增厚(40%)。结论花瓣样强化是肾结核特征性的CT表现,再综合分析影像其它特征,可以对肾结核作出正确诊断。

Objective To investigate CT features of renal tuberculosis. Methods Twenty cases of pathologically and clinically proved renal tuberculosis were retrospectively analyzed. There are nine male and eleven femal patients. Clinical manifestation included frequency of urination, urgency of urination, haematuria, and pain in urination. Two of those patients had hidden symptom. All these patients were examined by spiral computed tomography. Plain scan, arterial phase, venous phase and delayed phase of urinary tract images images were obtained. Results Main features of renal tuberculosis were hydronephrosis （80%）, petaling enhancement in venous phase （70%）, ambiguity of peri-renal fat （60%）, and thickness of tubal wall （40%）. Conclusion Petaling enhancement of venous phase was a characteristic feature of renal tuberculosis. It would get accuracy diagnosis, when other features of CT images were analyzed comprehensively.
