瑞芬太尼引起呼吸抑制时的靶控血浆浓度

目的观察瑞芬太尼靶控输注（TCI）时患者呼吸抑制的50%有效靶控血浆浓度（CP50）。方法择期硬膜外-腰麻联合麻醉患者80例（ASAⅠ-Ⅱ）,应用TCI靶控输注系统,按不同血浆靶控浓度随机分为8组（n=10）：Ⅰ组（1.0μg·L^-1、Ⅱ组（1.5μg·L^-1、Ⅲ组（2.0μg·L^-1、Ⅳ组（2.5μg·L^-1、Ⅴ组（3.0μg·L^-1、Ⅵ组（3.5μg·L^-1、Ⅶ组（4.0μg·L^-1、Ⅷ组（4.5μg·L^-1。应用半数效量序贯法计算瑞芬太尼TCI时患者呼吸抑制的CP50及其95%可信区间（95%CI）。结果瑞芬太尼TCI时患者呼吸抑制的CP50为2.48μg·L^-1CP50的95%CI为2.24-2.74μg·L^-1结论瑞芬太尼TCI时患者呼吸抑制的CP50为2.

Objective To investigate the half-effective target plasma concentration （ CP50 ） of remifentanil for respiratory depression Methods Eighty patients of ASA status Ⅰ-Ⅱ undergoing combined spinal epidural （ CSE ） anesthesia were randomly divided into eight groups （ n = 10 ） according to the target plasma concentration of remifentanil （ target plasma concentration of 1.0μg·L^-1,1.5 μg·L^-1,2.0 μg·L^-1,2.5μg·L^-1,3.0μg·L^-1,3.5μg·L^-1,4.0 μg·L^-1, 4.5μg·L^-1）. Cp50 and 95% confidence interval （95% CI）causing respiratory depression were determined by probit analysis. Results The CP50 of remifentanil causing respiratory depression was 2.48μg·L^-1, and 95% CI was 2.24 - 2.73μg·L^-1. Conclusion The Cp50 of remifentanil causing respiratory depression was 2.48μg·L^-1.
