摘要 目的:探讨急性化脓性膝关节炎关节软骨损伤的MRI表现。方法:分析16例急性化脓性膝关节炎患者的MRI表现,重点观察关节软骨改变,与关节镜手术或关节切开手术所见对照。结果:16例病变关节的关节软骨MRI均清楚显示,其中4例关节软骨MRI表现正常(0级),12例见软骨信号异常和(或)软骨变薄、软骨表面不光滑改变(Ⅰ~Ⅱ级),其中出现软骨局部全层缺如者2例(Ⅲ级),与手术对照具有较好的一致性。同时,MRI可发现软骨下骨水肿、关节周围软组织肿胀及关节积液等改变。结论:MRI能够准确评价急性化脓性膝关节炎关节软骨的病损部位、程度,对临床治疗有重要的指导意义。 Objective:To evaluate the MRI manifestations of articular cartilaginous lesions in acute pyogenic arthritis of knee joint. Methods:MRI appearances of 16 patients with acute pyogenic arthritis of knee joint were analyzed, the cartilaginous changes were mainly studied and correlated with arthroscopy and arthrotomy findings. Results: Of the 16 patients having MRI,the articular cartilage were clearly showed. There were normal cartilage (Grade 0) in 4 patients,abnormal cartilage signal intensities and/or thinning or roughness of cartilage surface in 12 patients (Grade Ⅰ~Ⅱ ),and focal full-thickness disappearance of articular cartilage in 2 patients (Grade Ⅲ ), which were in consistence with the operation findings. Sub chondral bone edema, peri articular soft tissue swelling and hydrarthrosis could also be assessed on MR imaging. Conclusion:The position and degree of articular cartilaginous lesions in acute pyogenic arthritis of knee joint could be accurately revealed on MRI,which is useful in treatment guidance in clinical application.
