高血压合并腔隙性脑梗死血压动态变化的观察与分析

Feature of ambulatory blood pressureof the patients in essential hypertension with cerebral lacunar infarction

目的探讨原发性高血压(EH)合并腔隙性脑梗死(CLI)患者的血压动态变化特征。方法将57例Ⅲ期EH患者分为两组:30例合并CLI者为观察组,27例无CLI者为对照组。全部患者作血压动态变化的测定,比较两组间各血压动态变化值的差异。结果在合并CLI的EH患者中,夜间收缩压水平明显高于无CLI的Ⅲ期EH患者(P=0.025);在所有Ⅲ期EH患者中,无论有无CLI,其血压昼夜节律均明显异常(收缩压夜间下降率△SBP及舒张压夜间血压下降率△DBP均<10%),但合并CLI者△SBP及△DBP均显著低于无CLI者(P值分别为0.008和0.035)。结论夜间收缩压和夜间血压下降率与CLI的发生有密切关系。

Objective To explore the feature of amjbulatory blood pressure of the patients in essential hypertension （EH） with cerebral lacunar infarction （CLI）.Methods 57 Ⅲ stage EH patients were divided into observe group （with CLI,30 patients） and control group （without CLI,27 patients）.All were studied with ambulatory blood pressure mornitoring and contrast the difference of parameter of ambulatory blood pressure in two group. Results The SBP at night time （nSBP） was significantly higher in patients with CLI than in Ⅲ stage EH patients without CLI P 〈 0.05; The circadian rhythm of blood pressure significantly abnormal in all In stage EH patients whatever with or without CLI （ the decrease rate of blood pressure in night time of SBP and DBP,△SBP and/XDBP were less than 10% ） ,but both the △SBP and △DBP were significantly lower in patients with CLI than in patients without CLI （ P 〈 0.01 and P 〈 0. 05）. Conclusion The findings suggest that occur of CLI in EH patients have accompanies with the nSBP and the decrease rate of blood pressure in night time.
