心尖肥厚型心肌病的诊断分析

Analysis of diagnosis for apical hypertrophic cardiomyopathy

目的:探讨心尖肥厚型心肌病(AHCM)辅助检查特点,以探求合理的诊断方法。方法:总结1998年7月～2008年10月住院的27例AHCM患者的心电图、超声心动图、冠状动脉及左心室造影等检查特征。结果:所有患者心电图显示胸导联V3-V5R波振幅增高伴T波对称性深倒置;22例AHCM患者通过心脏超声诊断明确,13例通过左心室造影确诊AHCM,无1例合并冠心病。结论:心电图特征性改变是心尖肥厚型心肌病的首要诊断依据,超声心动图是诊断AHCM的主要方法,左心室造影是诊断AHCM的重要方法。

Objective： To discuss characters of adjuvant inspection for apical hypertrophic cardiomyopathy （AHCM） and explore reasonable diagnosis method. Methods： Summarized characters of inspections for 27 cases with AHCM from July 1998 to October 2008, including electrocardiography, echocardiography, coronary and left ventriculograpgy and so on. Results： Electrocardiography showed wave amplitude of V3-V5 in chest leads increased and was accompanied by symmetry and deeply inverted T wave in all patients. 22 cases were confirmed by echocardiography, 13 by left ventriculography, and no case with coronary heart disease. Conclusion： Characteristic changes of electrocardiography provide primary evidence for diagnosing AHCM. Besides, echocardiography and left ventriculography are important diagnosis methods.
