多囊卵巢综合征患者血清脂联素与胰岛素抵抗相关性研究

Investigation on the correlation between adiponectin in serum and insulin resistance in women with polycystic ovary syndrome

摘要 目的:探讨多囊卵巢综合征(PCOS)患者血清脂联素(APN)与胰岛素抵抗(IR)的相关性。方法:以PCOS患者58例为研究组,同期非PCOS患者37例为对照组,按照体重指数(BMI)均分为BMI≥23、BMI<23两组,按照胰岛素抵抗指数(HOMA-IR)将研究组再分为胰岛素抵抗组和非胰岛素抵抗组,测定血清APN、胰岛素、性激素。结果:①研究组血清APN水平低于对照组(P=0.001),空腹胰岛素水平(Fins)、HOMA-IR高于对照组(P均<0.001)。②BMI与HOMA-IR呈正相关(r=0.544,P<0.001),与血清APN水平呈负相关(r=-0.392,P<0.001),研究组和对照组的BMI≥23组的血清APN水平均低于BMI<23组(P=0.001,P<0.05)。③研究组血清APN水平与HOMA-IR呈负相关(r=-0.346,P<0.001),控制BMI后血清APN水平与HOMA-IR无明显相关性(r=-0.240,P>0.05)。结论:PCOS患者存在明显胰岛素抵抗和低脂联素血症,血清APN水平降低与PCOS患者肥胖相关的胰岛素抵抗有关。 Objective: To explore the correlation between adiponectin in serum and insulin resistance in women with polycystic ovary syndrome (PCOS) . Methods: 58 women diagnosed as PCOS (case group) and 37 healthy women (control group) were selected and divided into BMI≥23 group and BMI 〈 23 group respectively according to the body mass index, case group was divided into insulin resistance group and non - insulin resistance group according to the insulin resistance index ( HOMA - IR) . The levels of adiponectin, insulin and sex hormones in serum were detected in four groups. Results: ①The level of adiponectin in case group was lower than that in control group (P =0. 001 ), while the levels of fasting insulin and HOMA - IR were higher than those in control group (P 〈0. 001 ) . ②There was positive correlation between BMI and HOMA - IR ( r = 0. 544, P 〈 0. 001 ), but there was negative correlation between BMI and adiponectin (r=-0.392, P〈0. 001), the BMI≥ 23 groups were lower than levels of adiponectin in serum of those of BMI 〈 23 group in case group and control group ( P = 0. 001, P 〈 0. 05 ) . ③There was negative correlation between adiponectin and HOMA - IR in case group ( r = - 0. 346, P 〈 0. 001 ), but after controlling the influennce of BMI, the significance disappeared ( r = - 0. 240, P 〉 0.05 ) . Conclusion: Insulin resistance and hypoadiponectin exist in PCOS patients and hypoadiponectin is related to obesity - related insulin resistance in PCOS patients.
