摘要
目的探讨骨栓的最佳植入方法。方法对1例34岁男性Ⅱ期股骨头缺血坏死患者行病变部位螺旋CT断层扫描。将得到的二维图像数据用M im ics软件系统处理并输入Pro ENG INEER W ILDFIRE 4.0进行模型构建,利用ABAQUS建立股骨上段三维有限元同构模型,模拟骨栓在不同位置施加4种不同载荷时股骨头内应力及应变情况,计算出骨栓放置的最佳位置。结果骨栓分别与压力骨小梁纵轴成30°、20°、10°、0°四个角度放置时,随着角度的减小在骨栓上出现的应力集中越来越明显,且到0°时达到最大。结论骨栓的植入能提高股骨坏死区的骨强度,而只有当骨栓按照压力骨小梁方向放置时该种效果才能达到最佳。
Objective To explore the optimal method for the implantation of bone-bolt. Methods One case with femur head necrosis in stage II was scaned by spinal CT, then the two-dimensional image was dealed with Mimics software and inputted to Pro-engineer wildfire 4.0. ABAQUS systems was used to build up three-dimensional finite element model of the upper femur,then the bone-bolt in different locations on femoral head were simulated to find the optimal location. Results The bone bolt could stress the femoral head from coUapse with the angles between the bone bolt and pressure trabecula decrease from 30° ,20°, 10° ,0° ,the tension concentration increased gradually ,and reaching a maximum at 0°. Conclusion The implantation of bone -bolt is able to improve the necrosis intensity,and the optimal effect can be obtained when placed according to prcssure trabecula direction.
