高危早产儿与高危足月儿脑性瘫痪的病残程度及脑干听觉诱发电位异常的对比研究 被引量:2

摘要 目的探讨高危早产儿与高危足月儿脑性瘫痪(脑瘫)及脑干听觉诱发电位的异常率进行对比研究。方法在脑瘫门诊选择10d^2个月早产儿出生前后有高危出生史为研究组188例,男107例,女81例;同期选择有高危出生史的足月儿97例为对照组,男58例,女41例。年龄0~7岁,平均(1.9岁±0.35)岁。2组脑瘫病残程度及脑干听觉诱发电位异常率进行研究。结果研究组脑瘫病残程度,轻度11例(5.8%),中度62例(32.9%),重度115例(61.17%)。对照组97例,轻度37例(38.14%),中度29例(29.87%),重度31例(31.95%)。2组重度所占比例,经统计学分析,2χ=4.35,P<0.05,研究组重度比例明显高于对照组。脑干听觉诱发电位:研究组188例,正常26例(13.82%),轻度33例(17.53%),中度40例(21.27%),重度89例(47.30%),异常率为89.11%。对照组:正常者36例(36.08%),轻度22例(22.68%),中度24例(24.47%),重度15例(15.46%),异常率为62.88%。2组经统计学分析,2χ=20.37,P<0.05,异常率明显大于对照组。结论高危早产儿、脑瘫的致残程度比足月儿病残程度重,脑干听觉诱发电位比足月儿异常率高。 Objective To investigate CP invalidity degree and brainstem auditory evoked potential abnormal rate between high-risk premature infants and high-risk term infants. Methods One hundred and eighty-eight cases were chose in CP clinic who was 10 days to 2 months premature infants and had a high risk birth history for study group, male of 107 cases and female of 81 cases, 97 cases of term infants cases were chose as control group, male of 58 cases, female of 41 cases. The age was 0- 7 years old , the average age was (1.9 ± 0.35)years old. The invalidity degree and brainstem auditory evoked potential abnormal rate between the two groups were investigated. Results Invalidity degrees in study group were that mild of 11 cases (5.1%), moderate of 62 cases(32.9%), severe of 115 cases (61.17%). In 97 patients of control group: mild of 37 cases (38. 14%), moderate of 29 cases(28.87%), severe of 31 cases(31. 95%). The severe proportion of the two groups had statis- tical significance,χ2 = 4.35, P〈0.05, and was obviously important than control group. Brainstem auditory evoked potentials in study group were that normal of 26 cases (13.82%) , mild of 33 cases (17.53 %), moderate of 40 cases (21.27%), severe of 89 cases(47.30%), the abnormality rate was 89.11% . Control group were that normal of 36 cases (36.08%), mild of 22 cases (22.68%), moderate of 24 cases (24.47%), severe of 15 cases (15.46%). Abnormality rate was 62.88%. The abnormal rate was significantly higher than that in control group. There was statistical significance in the two groups(χ^2 = 20.37, P〈0.05). Conclusion High-risk premature infants have a higher CP invalidity degree than term infants, and higher abnormality rate of brainstem auditory evoked potentials.
