摘要 ①目的探讨CT立体定向手术治疗高血压脑出血的临床应用价值。②方法对42例高血压脑出血病人行CT立体定向手术,抽吸血肿置管引流,有残余血肿的病人注入尿激酶溶解,观察病人血肿清除、并发症及功能恢复情况。③结果以CT显示残余血肿量0~5ml为拔管标准,拔管时间为12.0~168.0h,平均68.4h.本组40例病人随访90d后均有不同程度的功能恢复,且并发症少见。④结论CT立体定向手术治疗高血压脑出血是一种创伤小、操作简单、清除血肿彻底、手术安全。 Objective To explore the value of stereotactic neurosurgery in treatment of hypertensive cerebral hemorrhage. Methods 42 cases of hypertensive cerebral hemorrhage were treated with stereotactic neurosurgery. A pipe was used to suck the hemorrhage fluid and left there for continus drainage. Urokinase was injected through the pipe to melt hematoma. The clearance of hematoma, recovery of function and complication were observed. Results The pipe was removed when the volume of hematoma was less than 5 ml . The drainage time was 12—168h, averaging 68.4h. One case died(2.4%),one case failed in follow up study. The rest were followed up for more than 90 days,all recovered in various degrees and there was few complications. Conclusion Stereotactic neurosurgery with its virtues of less injury , easy to perform, safe and rapid complete clearance of nematoma is a good method for treatment of hypertensive cerebral hemorrhage.
