人工全膝关节置换术后感染的对策

Countermeasure of Infected Total Knee Arthroplasty

目的探讨治疗人工全膝关节置换术后感染的最佳对策。方法对1998年6月～2008年10月本院收治的5例人工全膝关节置换术后感染患者的临床资料进行回顾性分析。全部患者行彻底清创、使用有效抗生素或Ⅱ期置换术。结果感染均得到有效控制,平均随访46个月,均未出现感染复发,膝关节功能恢复满意。结论彻底清创、使用有效抗生素及Ⅱ期置换术是治疗假体感染的有效方法,但最好的对策还是加强预防。

Objective To explore the best countermeasure of infected total knee arthroplasty（TKA）. Methods From 1998 to 2008,5 cases patients with infection after TKAs were treated.Thorough debridement and effective antibiotic treatment were preseribed for 1 case,two-stage reimplantation for 4 case. Results 5 case were cured. All of the 6 patients were followed up, and on average for 46 months. No infection recurred.The function of knee recovered satisfiedly. Conclusion The effective management of infection after total knee arthroplasty includes thorough debridement, effective antibiotic treatment and two-stage reimplantation.But strengthening prophylaxis is still the best countermeasure.
