前列腺癌实时灰阶超声造影与MVD表达的相关性研究 被引量:5

Correlation study of real-time contrast-enhanced gray-scale ultrasonography and MVD in prostate cancer

摘要 目的超声造影定量评价前列腺结节血流灌注,并测定前列腺结节微血管密度(MVD),分析二者的相关性。方法采用对比脉冲序列超声成像技术及定量分析软件对43例经腹部前列腺超声发现49个前列腺异常结节的患者进行经腹部前列腺超声造影,并用免疫组织化学方法检测结节MVD,比较超声造影峰值强度与MVD之间的相关性。结果前列腺外腺恶性结节、外腺良性结节和内腺良性结节的MVD分别是48.13±11.36、15.38±1.51和25.17±4.42,前列腺外腺恶性结节的MVD高于外腺良性结节和内腺良性结节(P〈0.01)。结节造影峰值强度为(24.72±5.39)dB,MVD为34.35±15.78;前列腺结节超声造影的峰值强度与MVD之间呈正相关(r=0.845,P=0.000)。结论实时灰阶超声造影能客观反映前列腺病灶组织的微血管分布,与MVD相关性好,在观察前列腺病灶组织灌注的变化上具有潜在的临床应用价值。 Objective To quantitatively assess blood flow perfusion of prostatic nodules, and to determine microvessel density (MVD) of prostatic nodules, then to analyze their correlation. Methods Ttransabdominal prostate contrast- enhanced ultrasonography was performed on forty - nine abnormal prostatic nodules in 43 patients. MVD in the 49 prostatic nodules was detected by immunohistochemistry, then the correlation between peak intensity of contrast - enhanced ultrasound and MVD was compared. Results The MVD was 48.13±11.36, 15.38±1.51 and 25.17±4.42 in the extemal malignant nodules, extemal benign gland and inner benign gland of prostate respectively. MVD in external malignant gland of prostate was obviously higher than that in external benign gland and inner benign gland of prostate, there was significant difference among three groups ( P 〈 0.01 ). The peak intensity of contrast -enhanced uhrasonography was (24.72±5.39)dB, and the average of MVD was 34.35±15.78, there was positive correlation between PI and MVD ( r = 0. 845, P = 0. 000). Conclusion Real - time gray - scale contrast - enhanced ultrasonography can objectively retlect the distribution of MVD in prostatic nodules, and has good correlation with MVD. It also has potential clinical application value for observing the changes of prostatic nodules.
