摘要
目的探讨经导管动脉栓塞术(transcatheter arterial embolization,TAE)治疗难治性产后大出血的安全性和有效性。方法对16例难治性产后大出血(经阴道自然分娩3例,剖宫产13例)进行超选择性双侧髂内动脉或子宫动脉数字减影血管造影(digital subtraction angiography,DSA)和TAE治疗。结果本组均经股动脉插管进行DSA并应用明胶海绵栓塞,成功率100%,栓塞治疗后迅速止血,止血有效率100%,无栓塞相关并发症发生。结论经导管动脉栓塞术治疗难治性产后大出血安全、有效。
Objective To evaluate the safety and efficacy of transcatheter arterial embolization （TAE） in patients with postpartum hemorrhage. Methods 16 consecutive patients （3 with vaginal and 13 with cesarean delivery） with severe postpartum hemorrhage（PPn） underwent digital subtraction angiography（DSA） and TAE. Results Embolizafion was perfomaed by transfemoral arterial catheterization. Pieces of absoibable gelatin sponge were used in all eases. Optimal bleeding control was achieved in all eases, and no severe complications were observed. Conclusion TAE in patients with PPH is a safe and effective method.
