精神分裂症患者述情障碍与自测健康状况及抑郁的关系

A study on the relationship between alexithymia and self-related health condition, depressive mood in schizophrenics

目的了解住院精神分裂症患者述情障碍与自测健康状况及抑郁情绪的关系。方法采用白拟一般情况问卷、多伦多述情量表（TAS）、自测健康状况评定量表（SRHMS）、自评抑郁量表（SDS）对320例住院的精神分裂症患者与120名健康志愿者（对照组）进行评定，并进行相关与回归分析。结果（1）除TAS因子Ⅱ，精神分裂症组TAS总分及各因子分均显著高于对照组[依次为（73．53±10．38）分、（68．56±8．24）分；（17．61±4．67）分，（17．01±2．17）分；（15．25±4．33）分、（13．47±4．01）分；（19．65±4．40）分，（18．15±3．55）分；P〈0．05或P〈0．01）]。（2）精神分裂症患者TAS的多个维度与SRHMS、SDS的多个纬度呈显著相关（r=0．113～0．400，P〈0．05或P〈0．01）。（3）SDS的躯体障碍因子、受教育年限、SRHMS的心理症状与负向情绪因子等依次进入精神分裂症TAS总分的回归方程。结论住院精神分裂症存在明显的述情障碍，躯体障碍、受教育年限、健康状况等是其述情障碍严重程度的重要影响因素。

Objective To learn the relationship between alexithymia and self-related health condition,depressive mood in hospitalized schizophrenics. Methods 320 schizophrenics and 120 healthy controls were assessed with a self-developed questionnaire, the Toronto Alexithymia Scale （TAS） , Self-related Health Measurement Scale Version 1.0 （SRHMS） and Self-Rating Depression Scale （SDS）. Correlation and regression analysis were carried out. Results Compared with controls, schizophrenics showed significantly higher scores in the total score of TAS,TAS factor Ⅰ,Ⅱ and Ⅳ [total score（75.53 ± 10. 38） vs （68.56 ±5.24） ,TAS Ⅰ score（ 17.61 ±4. 6） vs （17.01 ±2.17） ,TAS Ⅱscore（15.25 ±4.33） vs （13.47 ±4.01） ,TASⅢscore（19.65 ±4.40） vs （18.15 ± 3.55 ） , P 〈 0.05 or P〈 0.01 ]. The most dimensionalities of TAS showed significant correlation to most dimensionalities of SRHMS and SDS （r = 0.113 - 0.400, P 〈0.05 orP 〈0.01 ）. The somatic disorder factor of SDS, education years and the negative mood factor of SRHMS entered the regression equation for the total score of TAS in schizophrenics by turns. Conclusion Schizophrenics suffered from significant alexithymia. The total score of SDS ,whether or not knowing the diagnosis and health status were responsible for alexithymia in hospitalized schizophrenics.
