真皮脂肪瓣充填结合牵引矫治乳头内陷

Correcting inverted nipple by inserting flap of dermis and fat and Traction

目的:介绍一种真皮脂肪瓣充填结合牵引矫治乳头内陷的方法。方法:①形成双侧真皮脂肪瓣作乳头下组织移植填充;②术中自制支架I牵引乳头10天;③术后第14天用自制支架II,缝线胶布继续牵引2个月。结果:总结了2004年5月～2008年12月收治的共44例患者,66只乳头。术后46只乳头得到随防1～3年,有3只乳头由于患者对胶布过敏,未使用缝线胶布外牵引,乳头中心部分回缩,但不影响哺乳,其余经使用缝线胶布外牵引,乳头均美观、挺拔,无复发;23例女性产后哺乳均正常,未发生乳汁潴留或乳腺炎。结论:真皮脂肪瓣充填结合牵引矫治中、重度乳头内陷,方法可靠,无复发。

Objective To introduce correcting inverted nipple by inserting flap of dermis and fat and traction. Methods ① Forming bilateral adipo-dermal flaps as organization under nipple to transplant and insert. ②Doing stentl,making nipple in traction for 10 days. ③After 14 days doing stent Ⅱ, making nipple in traction for 2 months with plasters that sutured. Results Cases that healed 44 patients （66 nipple） were summarized from May 2004 to December 2008. and with a routine follow-up of 1 to 3 years, 3 nipples recurrenced and the center retracted because of lacking traction that because they have an allergy to plasters, but don＇t impact on breast-feeding. And the other were treated effectively, 23 women can breast-feed and don＇t suffer from retention of milk and mastitis. Conclusions The most serious inverted nipples were corrected effectively by inserting flap of dermis and fat and traction.
