腹腔镜下子宫肌瘤剔除术132例临床分析

A clinical analysis of 132 cases of laparoscopic myomectomy

目的探讨腹腔镜下子宫肌瘤剔除术的治疗效果和影响因素。方法选择2006年7月。2008年12月在石河子大学医学院第一附属医院妇科行腹腔镜下子宫肌瘤剔除术的132例病例进行回顾性分析。结果全部手术采用腹腔镜下缝合止血技术，平均手术时间为59．29±24．56min，平均术中出血量为54．67±25．43mL，平均住院天数为5．17±1．65天，术后病率为0，无手术并发症出现。术中出血量和手术时间主要与肌瘤生长部位及其大小有关。结论腹腔镜下子宫肌瘤剔除术具有微创手术的优点，是一种安全有效且保留子宫的手术方法。

Objective To investigate therapeutic effect and influcing factors of myomectomy under laparoseopy. Methods The clinical data of 132 patients who received laparoseopie myomectomy in Department of Gynecology and Obstetrics, The First Affiliated Hospital of Medical College,Shihezi University over a period from July, 2006 to December, 2008 were analyzed retrospectively. Results 78 women suffered from single hysteromyoma, including 41 eases of intramural hysteromyoma, 32 cases of sudserous hysteromyoma, 5 eases of broad ligament leiomyoma. 54 women suffered from multiple hysteromyomas. 19 patients had history of abdominal surgery and 5 patients had two histories of abdominal surgery. All cases were performed on laparoseopie suture for hemostasis. The mean operation time was 59. 29 ± 24.56 minutes, the average intraoperative blood loss was 54.67 ± 25.43mL, the average time of postoperative hospitalization was 5.17 ± 1.65 days, the postoperative pyrexia rate was 0%. No operative complications were found. The intraoperative blood loss and operation time were related to location and size of hysteromyomas. Conclusion Laparoscopie myomeetomy has an advantages of minimal invassiveness, so, it is an effective procedures to preserve uterus of the patient.
