摘要
多普勒超声脐血流测定具有操作方便、对胎儿无损害等特点,是估计胎儿宫内情况的一种有效方法,在产科高危妊娠的监测中应用广泛。脐动脉舒张末期血流缺失或返流是脐动脉血流阻力异常增高的特有表现。出现脐动脉舒张末期血流的缺失或返流时,高危妊娠围产儿死亡率及发病率增高。该文就脐动脉舒张末期血流缺失与高危妊娠的研究进展作以综述,探讨脐动脉舒张末期血流缺失病因、病理、治疗方案及与国产儿结局的关系,阐述了测量脐动脉和大脑中动脉等血流参数对于衡量胎儿宫内情况及指导临床处理的重要作用。
As an effective method to estimate intrauterine condition of fetus, measurement of umbilical arterial blood flow by ultrasound doppler is used widely in monitoring of obstetric high-risk pregnancy because of its superiors such as being easy to operate and harmless ness to the fetus. Absent or refluxed end-diastolic velocity (AEDV or REDV) in the umbilical artery is the unique performance of abnormally increased resistance of blood flow in the umbilical artery. In ease of AEDV or REDV in the umbilical artery, the perinatal mortality rate and morbidity rate in high-risk pregnancy are increased. This review summarized progress in research on relationship between AEDV or REDV in the umbilical artery and high-risk pregnancy, explored relationships between etiology, pathology, treatment programs of AEDV or REDV in the umbilical artery and perinatal outcomes and expounded importance of measuring hemodynamic parameters of umbilical artery and middle cerebral artery for estimating fetal intrauterine condition and instructing clinical treatment.
