摘要
目的探讨影响子宫内膜癌患者生存及预后的因素。方法回顾性总结中山大学肿瘤医院妇科自2000年1月至2004年1月收治的经手术治疗的267例子宫内膜癌患者的临床病理资料和随访结果,进行生存分析及预后的影响因素分析。结果267例患者3年生存率、5年生存率分别为90.8%、88.1%,单因素分析显示:手术-病理分期、病理分级、组织学类型、肌层浸润深度、淋巴结转移、附件转移、术后是否治疗、ER和PR表达与预后有显著性相关(P<0.05)。经多因素分析后得出,手术-病理分期、病理分级、组织学类型、PR表达4个因素对子宫内膜癌患者的总生存率均产生显著性影响(P<0.05)。结论子宫内膜癌的5年生存率较高。手术-病理分期、病理分级、组织学类型、PR表达情况是影响预后的独立因素。
Objective To investigate the survival analysis of endometrialcarcinoma, and to explore its prognostic factors. Methods A total of 267 patients with endometrial carcinoma in our hospital from Jan. 2000 to Jan. 2004 were studied retrospectively. Prognostic factors were analyzed by univariate and multivariate analysis. Results The total 3 - year and 5 -year survival rate of 267 cases were 90.8% and 88.1%. The univariate analysis showed that operative stage, pathological type, histological grade, depth of muscularis invasion, lymph - node metastasis, adnexae metastasis, postoperation treatment, the expression of ER and PR significantly associated with the prognosis（P 〈 0. 05）. However, on multivariate analysis, only operative stage, pathological type, histological grade and the expression of PR significantly correlated with 5 - year overall survival（P 〈 0.05 ）. Conclusions The 5 - year survival rate of endometrial carcinoma is comparatively high. Operative stage, pathological type, histological grade and the expression of PR are independent prognostic factors of endometrial carcinoma.
