摘要
采用无创生物阻抗技术和同步胃电检测相结合的胃动力检测与评价新方法,对功能性消化不良病人进行初步的临床应用研究。同时提取体表胃电活动和机械运动信息,研究从胃电活动到机械收缩、蠕动的复杂胃动力过程,对采集信号提取时域、频域和变异系数等参数并进一步统计分析。功能性消化不良病人的胃运动正常节律百分比(PNF)和正常功率百分比(PNP)明显小于健康组,节律变异系数(FIC)和功率变异系数(PIC)大于健康组。功能性消化不良病人经一周用药后其EGG节律有较好的恢复,但反映胃机械运动的阻抗信号变化不大,提示经一周治疗的功能性消化不良病人,其胃动力的电学机制还没有耦合到机械收缩过程中,其胃动力功能还没有得到有效的改善。
A new method of combining electrical bioimpedance with synchronous EGG to measure and evaluate gastric motility has been proposed in this article. The functional dyspepsia was evaluated using the proposed method. Based on the cognition of electricity- mechanism compound system, the gastric motility information of both electricity and mechanism was extracted, the process from electrical activity to mechanical constraction and peristalsis was investigated. The parameters such as time, frequency domain and coefficient of variability were calculated. The results showed that the percentage of normal frequency and normal power of functional dyspepsia group were evidently less than that of control group, frequency instability coefficient and power instability coefficient of functional dyspepsia group were evidently bigger than that of normal group. EGG rhythm of the patients with functional dyspepsia recovered better after treated for one week, while the impedance gastric motility signal that reflects the mechanical constraction of stomach was little changed. This suggests that for the patients with functional dyspepsia after treated for one week, the electrical mechanism of gastric motility has not been coupled into the process of mechanical constraetion and peristalsis, the gastric motility function has not been improved effectively.
