瞬目反射、脑干听觉诱发电位、经颅多普勒超声对椎基底动脉供血不足的诊断价值

The diagnostic value of BR,BAEP and TCD on vertebrobasilar artery insufficiency.

目的：观察瞬目反射（ＢＲ）、脑干听觉诱发电位（ＢＡＥＰ）、经颅多普勒超声（ＴＣＤ）对椎基底动脉供血不足（ＶＢＩ）的诊断意义。方法：对４１例已经临床确诊的ＶＢＩ病人在间歇期进行ＢＲ、ＢＡＥＰ及ＴＣＤ检查。结果：ＴＣＤ、ＢＲ、ＢＡＥＰ异常率分别为８３％、８０％、６８％。ＢＲ提示脑桥损害１例、延髓损害１５例、广泛性脑干损害１７例；ＢＡＥＰ发现内耳听力减退２０例、脑干病变１３例；ＴＣＤ发现多血管流速异常１４例、一支流速改变１６例，有或伴有血管张力异常１３例。结论：由于ＢＲ、ＢＡＥＰ反射路径不同，检测结果不尽一致。联合检查有助于对病损部位进行定位。

Objectives:To investigate the value of blink reflex (BR),brainstem auditory evoked potentials(BAEP) and transcranial doppler(TCD) on vertebrobasilar artery insufficiency(VBI) patients in the intMethods:forty-one cases confirmed by clincal standards were examined by the three methods.Results: The abnormality rate of TCD,BR and BAEP was 83%,80%and 68% respectively.By BR,one case with lesions in pon,15 cases with lesions in medulla and 17 cases with lesions in broad brain-stem;By ABEP,23 cases with lesions in peripheral,13 cases with lesions in brain-stem;By TCD,changed MFV in multiple vessels were found on 14 cases,only one vessel on 16 cases;13 cases had or acompanied with the abnormal vessel tone.Conclusions:Owing to different reflex courses,the results of BR and BAEP were not consistent compeletly.By combination of the two methods,lesions could be easily located.TCD examination helped to find some vascular causes.It was suggested to regard these three methods as routine examinations on VBI.
