The Effect of Physiological Curvature on Restoration of Vertebral Body and Spinal Canal Decompression in Thoracolumbar Vertebral Fracture

摘要 本文探讨脊柱的生理弯曲与椎体高度的恢复对胸腰椎骨折病人神经管道恢复的影响。通过对56例术后病人,手术前后X线片,CT片的观测,提示:沿脊柱生理弯曲的轴向撑开力使椎体复位,椎管能获得最好的减压;椎体复位差,脊柱生理弯曲未恢复、轴向撑开后纵韧带紧张虽能使骨块部分回纳,但效果明显较前者差(P〈0.01)。预计椎体难以撑开复位的病例,椎板开窗去除致压物是可取的。 The effect of physiological curvature on restoration of the height of vertebral body the neural canal in the patients with thoracolumbar fracture was explored. We surveyed 56 patients and their manifestion of preoperation and post-operation X - ray and CT. The results indicated that the axial bracing load on spinal physiological curvature made the vertebral body the best reposition and spinal canal decompression. When the vertebral body was reposited poorly or the spinal physiological curvature was not restored, the tight posterior longitudinal ligament that was axial braced, it would make bone fragments reposit partially. But the effect was sinificantly poorer than the former' s. If it is difficult to reposite the fracture vertebral body by axial bracing load, it is feasible to make fenestration on lamina to removel the compressor.
