摘要
目的探讨CT灌注参数评价肺恶性肿瘤血管功能状态的价值。方法对23例肺恶性肿瘤患者行64层螺旋CT灌注扫描,以CT灌注成像处理软件进行处理。感兴趣区的选择采用高灌注区均值和最大层面全肿瘤区域两种方式,分别获得血流量(BF)、血容量(BV)、表面渗透性(PS)等灌注参数,绘制时间-密度曲线,并测量肿瘤的CT强化峰值(PH),对各灌注参数值及PH进行相关性分析。结果①肺恶性肿瘤的PH与CT灌注成像BV的高灌注区均值、全肿瘤均值及BF的全肿瘤均值均呈显著的正相关;②肺恶性肿瘤的PH与BF的高灌注区均值及PS无显著相关性。结论肺恶性肿瘤多层螺旋CT灌注成像大部分灌注参数与PH有共同的组织学基础,呈显著的正相关,在评价肺恶性肿瘤的血管功能状态方面有重要价值。
Objective To assess the value of CT perfusion in evaluating the function of blood vessel in lung malignancy. Methods A total of 23 patients with lung malignancy underwent perfusion scan with 64-slice spiral CT scanner. The perfusion parameters of blood flow (BF), blood volume (BV) and permeability were analyzed using the ROI extractions including mean of hot region and mean of whole tumor. Then the time-intensity curve was drawn and peak height (PH) was measured. Correlation analysis was processed with SPSS 10.0. Results (1)Significant positive correlation was found between PH and BV (both ROI extractions), PH and BF (mean of whole tumor); (2)No significant positive correlation was found between PH and BF (mean of hot region), PH and PS (both ROI extractions). Conclusion Most of CT perfusion parameters such as BV have something in common with PH histologically and show significant positive correlation with it, and may have important value in evaluating the function of blood vessel in lung malignancy.
