螺旋CT多平面和曲面重建在低位胆道梗阻诊断中的应用 被引量:13

摘要 目的探讨多层螺旋CT多平面重建(MPR)和曲面重建(CPR)在低位胆道梗阻诊断中的临床应用和价值。方法90例低位胆道梗阻患者行薄层扫描,进行冠状面、矢状面及斜面的多平面重建和沿胆道或胰管路径的曲面重建,以手术、病理及临床随访结果为标准。结果MPR和CPR的定位诊断准确率均为100%;病因诊断准确率MPR为95.6%,CPR为93.3%。结论在低位胆道梗阻疾病的诊断中,MPR和CPR的定位诊断能力相同,病因诊断能力MPR略高于CPR。两种重建技术有各自的优缺点,在临床诊断中应相互结合、互为补充。 Objective To assess the clinical application and value of multislice CT muhiplanar reconstruction(MPR) and curved planar reformations (CPR)in diagnosis of low-set biliary obstruction. Methods Thin slice scans were performed to acquire images in ninety patients of low-set biliary obstruction. MPR images were performed on coronal, sagittal and incline plane. CPR technique was used to reconstruct the images along the direction of pancreatic and bile ducts distribution. The surgery,pathology and clinical follow-up result were regarded as gold standard. Results The accuracy of MPR and CPR on localizing diagnosis were both 100%. The accuracy of qualitative diagnosis were 95. 6% and 93.3% respectively. Conclusion MPR is equal to CPR on localizing diagnosis of low-set biliary obstruction. On qualitative diagnosis abihty, MPR is little higher than CPR. The two reconstruction techniques have respective predominance and limitation, they should combine and supply each other in clinical diagnosis.
