青岛地区不同医疗机构性病服务质量现状与对策

VENEREAL DISEASE SERVICES IN DIFFERENT MEDICAL INSTITUTIONS IN QINGDAO:QUALITY AND STRATEGY

目的对青岛地区不同性病医疗机构服务质量进行调查,为改善其服务提出建议。方法选择该地区所有公立性病专科医院和部分公立综合医院皮肤科、泌尿科和妇产科及部分民营医院的性病科作为被调查机构,采用直接观察医生法、访谈医生法、访谈性病病人法及假扮病人法调查性病服务质量,并进行比较。结果共观察70名公立医院医生处理90例性病病例,按照WHO/GPA方案计算得到预防指标6(PI-6)为63.3%,预防指标7(PI-7)为17.8%。观察27名民营医院医生处理57例性病病例,其PI-6为8.7%,PI-7为1.8%。公立医院与民营医院在性病临床及预防服务等方面比较,差异有显著性(χ2=4.49～46.24,P<0.05)。结论公立医院医生重视临床、忽视预防,民营医院医生在这两方面的工作都较差。应加强对医务人员的专业培训,进一步提高性病服务质量。

Objective To investigate the quality of different medical institutions of STD therapy in Qingdao and raise suggestions to improve the services. Methods The survey was eondueted on some STD clinics and departments of dermatology, urology, obstetrics and gynecology at government and private hospitals through direct observation, visiting doctors, and interview with patients and saw a doctor disguised as a patient. Results A total of 70 doctors managing 90 STD cases at government hospitals were observed, the overall rate for PI-6 was 63.3 %, the score for PI-7 was 17.8 %, based on WHO/GPA program; and 27 doctors who treated 57 STD patients at private hospitals were observed as well, the PI-6 was 8. 7%, and PI-7 was 1.8%. Compared with government hospitals, the difference was significant in terms of treatment and prevention （χ^2 = 4.49- 46.24, P〈0. 05）. Conclusion The doctors at government hospitals think highly of treatment and disregard prevention; and those at private hospitals pay little attention to both of above two aspects. It is necessary to reinforce professional training for them to improve the quality of STD serviee.
