经侧裂岛叶入路微侵袭手术治疗高血压性壳核出血的临床研究

Clinical Study of the Minimally Invasive Surgical Treatment of Hypertensive Putamianal Hematomas via Transsylvian Transinsular Approach

目的:探讨经侧裂岛叶入路微侵袭手术对高血压性壳核出血患者的治疗价值。方法:回顾性分析65例HPH患者的临床资料,其中51例患者接受经侧裂岛叶入路开颅手术治疗,14例采用保守治疗。结果:手术组平均住院日、主要并发症发生率、总体死亡率均低于保守治疗组。随访6月时,手术组恢复良好率高于对照组。结论:经侧裂岛叶入路手术治疗可改善高血压性壳核出血患者临床预后。

Objective： To investigate the therapeutic value of the minimally invasive surgical treatment of hypertensive putamianat hematomas via transsylvian transinsular approach. Methods： The data of 65 HPH cases were analyzed retrospectively, among which 51 cases were surgically treated via transsylvian transinsular approach, while 14 cases were treated conservatively. Results： The mean duration of hospitalization, the morbility of the main complications, total mortality of the surgical treatment group is shorter/less than the conservative treatment group. After 6-month follow-up, the well-recovered rate of surgical group is higher than the control group. Conclusions： Surgical treatment via transsylvian transinsular approach of HPH results in improved outcome as compared to conservative treatment.
