摘要
目的:通过研究胃癌及癌前病变组织中细胞表面粘附分子CD24的表达,探讨CD24蛋白与胃癌临床特征的关系及胃粘膜癌前病变及癌变的分子机制。方法:应用单克隆抗体S-P免疫组化检测280例不同胃组织(包括正常胃粘膜32例、慢性胃炎66例、肠化生胃粘膜58例、胃腺癌68例、癌旁组织44例、胃间质瘤12例)中CD24蛋白的表达。结果:CD24在正常胃粘膜、慢性胃炎中表达阳性率为5.8%,在胃间质瘤中亦无表达,在癌旁组织中表达阳性率为34.1%;CD24在胃癌组织中阳性表达率为80.8%,同肿瘤病理学分级、临床分期及淋巴结转移相关;在肠化生胃粘膜组织中表达阳性率为87.9%(51/58);肠型胃癌中CD24蛋白阳性表达率高于弥漫型胃癌,差异有统计学意义(P<0.05)。结论:CD24的表达与胃癌肿瘤病理学分级、临床分期及淋巴结转移有相关性,同时CD24的异常表达可能与胃癌的早期发生有关,CD24高表达胃癌其临床分期较晚,侵袭性高,细胞分化程度低,因而检测其表达强度有助于判断胃癌恶性程度及胃癌患者的预后。
Objective: In this study we examined the expression of cell surface adhesion molecule CD24 in gastric cancer and gastric precancerous change to approach the molecule mechanism of the precancerous lesion and gastric tumor. Methods: The expression pattern of CD24 in 68 gastric adenocarcinnmas, 32 normalgastric epithelias, 66 Chronic gastritis, 34 chronic atrophic gastritis, 58 intestinal metaplasias, 44 nearing cancer epitheliums, 12 gastric stromal tumors was analyzed by immunohistochemistry. Results:The expression of CD24 in normal gastric epithelia, nearing cancer epitheliums and gastric stromal tumors are negative ; the positive rates of CD24 expresion in chronic gastritis is 5.8% ; the positive rates of CD24 expresion in intestinal metaplasia and gastric adenocarcinoma are 80.8% and 87.9% and was associated with highstage tumor, tumor cell differentiation and lymph nodes invasion ; the positive rates of CD24 expresion in intestinal metaplasia epithelium is 87.9%, the positive rates of CD24 expresion in intestinal type was higher than those in the diffuse type gastric cancer ( P 〈 0.05 ). Conclusion: Cytoplasmic expression of CD24 was associated with high-stage tumor, tumor cell differentiation and lymph nodes invasion, The abnormal expression of CD24 was correlated with early stag of gastric cancer. Gastric cancer with high CD24 index have high invasion, low differentiation,and detection of CD24 expression in gastric cancer tissues can be useful in the determination of malignancy and prognosis.
