腹腔镜胆囊切除术预防胆管损伤1300例报告

Clinical analysis of bile duct injuries in laparoscopic cholecystectomy:with a report of 1 300 cases

目的:探讨腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)中预防胆管损伤的方法。方法:回顾分析1998年12月至2007年10月我院为1300例患者行LC的临床资料。结果:1275例手术成功,25例中转开腹,2例胆管损伤,余均无并发症发生。结论:LC术中规范操作,充分显露胆囊三角,重视解剖变异,观"颜色"、测"张力"、判"宽度"、看"间隙"是预防胆管损伤的重要措施。

Objective：To discuss the approaches to prevent bile duct injuries in laparoscopic cholecystectomy （LC）. Methods：The clinical data of 1 300 cases undergoing LC from Dec. 1998 to Oct. 2007 were collected and analyzed retrospectively. Results： One thousand two hundred and seventy-five cases accepted LC successfully, and 25 cases were converted to open surgery. Bile duct injuries occurred in 2 cases, and no death and other serious complications occurred. Conclusions：Normative operation, correct dissection of Calot＇s triangle and observation of anatomical variations, attention paid to the color, tension, width and gap of biliary system should be guaranteed to prevent the bile duct injuries in LC.
