食管癌住院患者家庭功能与家庭负担的研究及护理干预

Study on the family function and burthen for in-patient with esophagus cancer and its nursing intervention

目的:探讨食管癌住院患者家庭功能与家庭负担状况及相关因素,为提高整体护理服务质量提供数据支持。方法:采用家庭关怀度指数量表(APGAR量表)和家庭负担会谈量表(FBS量表)对240例食管癌住院患者的家庭进行测量和分析并给予护理干预。结果:本组147例(61.25%)家庭功能良好,58例(24.17%)家庭功能出现轻度障碍,35例(14.58%)家庭功能严重障碍。APGAR评分中亲密度得分最高,合作度得分最低。FBS问卷得分中家庭经济负担得分最高,家庭成员躯体健康得分最低。性别、付费方式、文化程度、居住地FBS得分比较,差异有显著性意义。家庭负担总分与家庭功能呈显著负相关。结论:食管癌患者住院治疗给患者家庭带来多维度的影响,造成家庭功能障碍,导致家庭负担增加。提示我们在工作中应强化整体护理理念,有针对性的为患者及其家属提供切实可行的干预措施,促进患者家庭功能健康发展,提高患者应对疾病的能力,改善患者预后,缩短住院天数,减轻患者家庭负担。

Objective：Study on the conditions and correlative factors of family function and burthen for in - patient with esophagus cancer , so as to provide data support for improving service quality of holistic nursing. Methods ： The family of 240 in - patients with esophagus cancer were investigated and analyzed by Family APGAR Index（APGAR） and Family Burden Scale of Disease（FBS） ,and to implement nursing intervention. Results： 147 （61.25%） patients has batter of family function ,58 （24.17%） patients experienced slightor burden of family function,35 （ 14.58% ） patients has serious burden of family function. The score of intimacy was the highestand the cooperation had the lowest score in the total score of APGAR,The family burden had the highest score and the physical health of their familymember had the lowest score in the total score of FBS. There was remarkable sense of FBS in sex,the way of pay,the level of culture and habitancy. A negative correlation was found between family function and total score of family burden. Conclusion ：Therapy of in - patients with esophagus cancer will not only substantially affect the patients physically andmentally, but also influence their family function inmanyways, leading to increased family burden. The results shows that nurse must enhance holistie nursing idea and provide targeted intervention measures for patients and family, so as to advance healthy development of family function, improve the ability of respond to disease and prognosis, shorten the length of stay in hospitaland reduce family burden.
