胃上皮样平滑肌瘤的外科治疗(附13例临床病理分析)

Surgical Treatment of Gastric Epithelioid LeiomyomaA Report of 13 Cases

１９８０年１月至１９９５年１２月间作者共收治胃上皮样平滑肌瘤１３例，均经手术切除，包括胃部分切除术（５例），胃大部切除术（４例），根治性胃大部切除术（４例）。作者认为上腹痛、呕血和／或黑便及腹部包块为主要临床表现。大体所见分为胃壁内、胃内、胃外三型。胃镜及活检、胃双对比造影、腹部Ｂ超及ＣＴ等对诊断本病有较大意义。手术切除是目前唯一有效的治疗手段。

From 1980 to 1995, 13 cases of gastric epithelioid leiomyoma were treated surgically. Partial gastrectomy was performed in 5 cases,subtotal gastrectomy in 4 and radical subtotal gastrectomy in 4. The main clinical manifestation of these patients included epigastralgia, hematomesis or melena and abdominal mass.Grossly,gastric epithelioid leiomyomas were classified as three patterns intramural, endogastric and extragastric. It is concluded that gastroscopy and biopsy, doublecontrast barium meal, abdominal Bultrasonic and CT examination are helpful for diagnosis and that complete surgical resection seems the unique effective method for treating this tumor because it is not sensitive to chemotherapy nor radiotherapy.
