淋巴瘤、多发性骨髓瘤患者血栓前状态的观察

Observation on prethrombotic state in patients withlymphoma and myeloma

目的：探讨淋巴瘤、多发性骨髓瘤患者有无血栓前状态，判断病情，以其指导治疗。方法：采用放射免疫法、发色底物法等对４０例淋巴瘤、２４例多发性骨髓瘤的血小板表面α－颗粒膜蛋白、凝血酶原、凝血酶、血管性假血友病因子及抗凝血酶Ⅲ进行了测定。结果：前三项指标淋巴瘤组分别为４２．５％、３０％和４５％，多发性骨髓瘤组分别为６２．５％、２０．５％和５８．３％患者较正常升高。少数晚期淋巴瘤抗凝血酶Ⅲ含量下降。晚期淋巴瘤较早期易发生血栓前状态。结论：淋巴浆细胞系统恶性肿瘤常伴血栓前状态。

Objective:Prethrombotic state in patients with lymphoma and multiple myeloma was studied in order to judge the condition and direct the treatment.Methods:Radioimmunoassay,chromogenic substrate,etc were used to determine molecule number of platelet surface αgranule membrane protein (GMP140),Prothrombin (FⅡ),von willebrand factor (VWF),thrombin and anti thrombin (ATⅢ) in 40 patients with lymphoma and in 24 patients with multile myeloma (MM).Results:GMP140,FⅡ and thrombin increased,being 42.5%,30%,45% respectively in patients with lymphoma and 62.5%,21.5%,58.3% in patients with MM.VWF increased in more than half patients.Antigen and activity of ATⅢ decreased in a few advanced lymphoma patients.Conclusion:These data suggest that lymphomaplasmacyte system malignant tumor are often associated with prethrombotic state.The severity of this state is closely related to period of disease.
