409例高血压病患者不同中医证型间心血管易损因子水平的变化 被引量:16

Analysis of Cardiovascular Vulnerable Factors Among 409 Cases of EH Patients with Different Pattrens of TCM

摘要 目的:探讨高血压病患者中医证型分布规律及常见心血管易损因子水平的变化。方法:对409例高血压病患者进行中医辨证分型、血压分级及危险分层;分别检测不同中医证型患者的高敏性C反应蛋白(hs-CRP)、高半胱氨酸(Hcy)、血管性血友病因子(vWF)、B型钠尿肽(BNP)的水平。结果:痰湿壅盛组血清Hcy、hs-CRP、BNP平均水平显著增高;夹痰瘀组血清Hcy、BNP水平显著高于夹瘀、夹风组;血浆vWF平均水平在阴阳两虚组最高;夹瘀组血浆vWF水平显著高于夹痰组。结论:阴阳两虚证、痰湿壅盛证及夹痰瘀患者易损因子水平较高,发生心血管事件的危险性相对较大。 To study the distribution law of traditional Chinese medicine (TCM) pattern in patients with essential hypertension(Eli) and the levels of the cardiovascular vulnerable factors among different TCM patterns. Methods: A total of 409 EH patients underwent pattern identification, hypertension grading and risk stratification. Meanwhile, the levels of cardiovascular vulnerable factors including homocysteine( Hey), high-sensitivity c-reactive protein( hs-CRP), yon willebrand factor(vWF) and B- type natriuretic peptide ( BNP), were detected respectively. Results: The levels of serum Hcy, hs-CRP and BNP were significantly higher in excess accumulation of phlegm-dampness pattern ; the levels of Hcy and BNP were significantly higher in the pattern with both phlegm and stasis than with stasis or wind ; the level of vWF in yin and yang deficiency pattern was the highest among all the patterns ; the level of vWF in the patterns with blood stasis was significantly higher than with phlegm. Conclusion : The levels of cardiovascular vulnerable factors of patients with yin and yang deficiency pattern, excess accumulation of phlegm-dampness pal- tern or concun'ing with phlegm and stasis are relatively higher than others, which may indicate higher risk of cardiovascular events.
