25例PICC异位的原因分析与护理对策

Reason analysis and nursing care of 25 cases of PICC ectopia

目的探索PICC异位的原因与护理对策。方法通过回顾性分析,对25例PICC异位的发生部位、置管途径、处理方法及结果进行研究、总结。结果PICC异位与静脉选择、置管长度的外测量方法、患者局部血管解剖变异、穿刺时患者体位、操作者送管技巧等因素有关。结论为了减少PICC异位的发生,置管部位宜选择右上肢、贵要静脉;传统外测量方法与送管时患者体位有待改进;多数PICC异位可以通过各种方法纠正。

Objective To explore the causes and nursing measures of eetopia of peripherally inserted central catheters （PICC）. Method The loeation,intubation pathway,treatment methods and outcomes in 25 cases of PICC ectopia were analyzed retrospectively. Result The main reasons of PICC ectopia were ranked as the following factors,such as the location of the vein,the measurement method of the length of outer catheter,the anatomic variation of the vascular,the patients＇ position during catheterization,and the techniques of the operator. Concblsion In order to reduce the incidence of PICC eetopia,it is suggested to choose the basilic vein of the right upper limb as the catheterization sit,improve the measurement method of the length of outer catheter as well as the patients＇ position. Most of the ectopic PICC can he corrected.
