运动训练对缓解期老年重度COPD患者肺功能和运动耐力的影响及其相关性研究 被引量:31

摘要 目的:探讨运动训练对缓解期老年重度慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者肺功能和运动耐力的影响及二者的相关性。方法:30例缓解期老年重度COPD住院患者随机分为对照组10例,运动训练组20例。运动训练组除常规内科治疗外进行步行训练和改编的呼吸体操训练,持续训练3个月。对照组10例,给予内科常规药物治疗。训练前后分别评价运动耐力(6MWD以及记步器步数)和Borg呼吸困难评分,同时测定肺功能。结果:COPD患者训练前后肺功能无明显差异(P均>0.05),运动训练组运动耐力(6MWD和记步器步数)明显高于训练前和对照组(P<0.01),Borg呼吸困难评分明显减轻(P<0.01)。增加的6min步行距离与肺功能之间无明显相关性。结论:运动训练可以提高老年重度COPD患者的运动耐力和减轻呼吸困难症状而对肺功能无明显改变。运动耐力的提高与肺功能之间无明显相关性。 Objective: To investigate the effects of exercises training on pulmonary function and exercises tolerance in elderly patients with stable severe chronic obstructive pulmonary disease (COPD). Method: Thirty cases of elder patients with stable severe COPD were randomly divided into two groups. The exercises training group consisted of 20 cases and the control group consisted of 10 cases. Both groups received medicine therapy. Meanwhile, the exercises training group received breathing retraining and walking training additionally for 3 months. The improvements of lung function, dyspnoea ratings (Borg scale), exercises tolerance (6MWD) were prospectively compared after 3 months exercises training program. Assessments were performed before and after the training program. Result: Ahhough lung function did not change significantly after exercises training, dyspnea and exercises tolerance improved significantly in exercises training group. Conclusion: Exercises training can markedly improve symotoms and exercises, endurance in elderly patients with stable severe COPD.
