脂联素基因多态性与胰岛素抵抗和2型糖尿病关系的研究 被引量:7

摘要 目的:探讨脂联素基因(APM-1)单核苷酸多态性(SNP)+276G/T在天津地区汉族人群中的分布情况及其与胰岛素抵抗和2型糖尿病的关系。方法:采用聚合酶链式反应-限制性片段长度多态性(PCR-RFLP)技术对196例2型糖尿病(T2DM)患者和165例糖耐量正常(NGT)人群APM-1基因SNP276进行分析,通过放免法测定血浆脂联素水平,并对各受试者相关临床指标进行检测。结果:(1)SNP276 3种基因型在T2DM组和NGT组间总体分布无统计学差异,但T2DM组中T等位基因频率显著低于NGT组(24.74%vs 32.42%,P<0.05)。(2)两组中,GG型BMI、FPG(仅见于T2DM组)、FINS、TG(仅见于NGT组)、HOMA-IR显著高于TT型,HDL-C、血浆脂联素水平显著低于TT型。(3)相关分析和多元逐步回归分析表明T2DM组中血浆脂联素水平与FPG和HOMA-IR呈显著负相关,HOMA-IR是影响脂联素水平最显著的因素。结论:APM-1基因SNP276与天津地区汉族人群2型糖尿病及胰岛素抵抗具有相关性。 Objective: To investigate the distribution of single-nucleotide polymorphism (SNP)+276G/T in adiponectin gene in people of Han nationality in Tianjin Area and its association with insulin resistance and type 2 diabetes mellitus(T2DM). Methods: 196 patients with T2DM and 165 subjects with normal glucose tolerance (NGT) were genotyped for SNP276 of APM-1 by PCR-RFLP. Plasma adiponectin concentrations and a homeostasis assessment of insulin resistance(HOMA-IR) and related clinical metabolic param- eters were measured. Results: (1) There was no statistically significant differences in genotype distributions of SNP in the two groups. However, the T allele frequency was significantly decreased in T2DM group (24.74% vs. 32.42%, P〈0.05). (2) In both NGT and T2DM groups, the plasma level of adiponection, BMI, FPG (only in T2DM group), FINS, TG(only in NGT group), HDL-C and HOMA-IR were significantly different in GG genotype carriers compared with tit genotype carriers. (3) Significant association was found between plasma adiponection concentration and FPG and HOMA-IR in T2DM group. Multiple stepwise regression analysis indicated that the HOMA-IR was the most important factor impact on plasma adiponection. Conclusion: SNP276 in adiponectin gene is associated with type 2 diabetes mellitus and insulin resistance.
