高屈曲与旋转平台假体术后膝关节屈曲度的比较研究 被引量:2

摘要 目的:对比PFC-RPF高屈曲度假体与传统PFC-RP旋转平台假体全膝关节置换术(TKA)后近期关节屈曲度的差异。方法:232例(346膝)行PFC-RPF和PFC-RP假体置换术的膝骨关节炎患者,随机分为10组,包括采用PFC-RPF的AⅠf/BⅠf/AⅡf/CⅡf/DⅡf组和采用PFC-RP的AⅠ/BⅠ/AⅡ/CⅡ/DⅡ组。摄取过屈及过伸位X片,分别测量胫股角,进行比较。结果:行单膝置换的AⅠf/AⅠ和BⅠf/BⅠ组间有显著性差异(P<0.05);双膝同时置换的AⅡf/AⅡ,CⅡf/CⅡ和DⅡf/DⅡ组间无显著性差异。结论:TKA术后2年内,单膝置换采用PFC-RPF者屈曲度优于PFC-RP,双膝同时置换时屈曲度差别不明显。 Objective: To compare the maximum flexion of knee joint after total knee arthroplasty ( TKA ) using high flexion prosthesis ( PFC-RPF ) and traditional rotating platform prothesis ( PFC-RP ). Methods: A total of 232 patients ( 346 knees ) with osteoarthritis underwent TKA using PFC-RPF or PFC-RP prosthesis. Patients were randomized to 10 groups. Groups A Ⅰ f/B Ⅰ f/A Ⅱ f/C Ⅱ f/D Ⅱ f underwent TKAusing PFC-RPF, while groups A Ⅰ /BⅠ /A Ⅱ /C Ⅱ /D Ⅱ underwent TKA using PFC-RP. The hyperextension and hyperinflexion of lateral knee joint were observed on radiograph and compared with the ibiofemoral angle. Results: A significant difference was observed in the maximum flexion of knee joint between groups A Ⅰ f / A Ⅰ and B Ⅰ f / BⅠ . However, no significant difference was found in the maximum flexion of knee joint between groups A Ⅱ f/A Ⅱ, C Ⅱ f/C Ⅱ and D Ⅱ f/D Ⅱ . Conclusion: The flexion of knee joint is better after unilateral knee replacement using PFC-RPF than with PFC-RP. However, the flexion of knee joint is not improved after bilateral knee replacement.
