吕仁和教授辨治糖尿病医案数据挖掘分析 被引量:16

摘要 目的应用数据库挖掘技术,总结吕仁和教授辨治糖尿病的学术思想和诊疗规律,同时评价"中医医案数据库"的应用前景。方法收集128例经吕教授诊治的糖尿病患者的医案,医案数据经过一致化处理之后,存储于"中医医案数据库"中,对糖尿病类型、分期、并发症、病因、病位等的出现几率及其关系进行统计分析。结果吕教授临床治疗的糖尿病患者以出现糖尿病并发症和伴发病的2型糖尿病患者为绝对主体;并发症以糖尿病肾病的出现几率最高,其次为糖尿病神经病变;"肥美之所发"的饮食因素和"怒则气上逆"的情绪因素是重要的中医病因;肾、肝为主要病位。结论"中医医案数据库"数据挖掘结果客观地反映了吕教授治疗糖尿病病的临床思路与经验,该技术适合中医医案的知识挖掘和学习,利于专家学术思想的总结。 Objective To sum up the academic thought and rules of diagnosis and treatment of Professor LV Ren-he for diabetes differentiation and treatment by applying database mining technology, and review the practical prospect of Database of Medical Records of Chinese Medicine. Methods The medical records of 128 patients with diabetes diagnosed and treated by Professor LV Ren-he were collected, and then the data were processed congruously and put into Database of Medical Records of Chinese Medicine. The occurrence rate of types, stages, complications, causes and locations of diabetes after syndrome differentiation based on TCM theory was calculated, and the relation among them was analyzed. Results The diabetes patients treated by Professor LV Ren-he were mostly those of type 2 diabetes with diabetes complications and concomitant diseases. The occurrence rate of diabetic nephropathy was the highest and that of diabetic neurepathy was secondly high. The diet factors, i.e. intaking too much greasy and delicacy food, and motion factors, such as upward of qi due to anger, were important in the onset of diabetes. The disease locations were mainly in kidney and liver. Conclusion The outcomes of data mining from Database of Medical Records of Chinese Medicine reflect objectively the clinical thinking and experience of Professor LV Ren-he, which can be applied in knowledge mining and study of TCM records, and summary of academic thought of experts.
