乳腺癌^(18)F-FDGPET/CT标准化摄取值与P53表达关系的研究

摘要 目的探讨18F-脱氧葡萄糖(FDG)PET/CT的最大标准化摄取值(SUVmax)与乳腺癌P53表达之间的关系。方法回顾性分析PET/CT诊断的乳腺癌患者,且计算病灶最大SUV(SUVmax);所有病例均手术后病理证实且进行免疫组化检查(P53检测)。结果118例乳腺癌患者中,P53阳性组占64.4%;P53阳性组与P53阴性组的年龄之间差别不具有统计学意义;P53阳性乳腺癌患者组平均SUVmax为5.06±1.47;而P53阴性乳腺癌患者组平均SU-Vmax为3.16±0.69,组间SUVmax比较差异具有统计学意义。本试验ROC曲线显示,SUVmax诊断P53表达正确率受试者工作特征(Receiver operating characteristic,ROC)曲线的曲线下面积达0.910±0.028,用SUVmax诊断P53表达具有统计学意义(P=0.000);根据ROC曲线发现SUV分界值为3.785,其诊断敏感性为85.5%,特异性为81.0%。结论利用18F-FDG PET/CT显像的SUVmax早期评价乳腺癌P53表达情况可作为一种新的评价方法,初步认为,将SUVmax>3.785作为乳腺癌P53表达的一个分界值有一定的科学意义。 Objective To investigate the relation of maximum standard uptake valve (SUVmax) of ^18F-fluorine-2- deoxyglucose (^18F-FDG) and the expression of P53 in breast carcinoma. Methods 118 cases with breast carcinoma was performed with PET/CT. The expression of P53 was detected with immunohistochemistry. Results P53 positive rate was 64.4% in 118 cases. The age in P53(+) and P53(-) were 49.45±9. 32 and 51.62 ±10.86 years old, respectively (t= 1. 142 ,P=0. 256). SUVmax in P53(+) and P53(-) were 5 . 06±1.47 and 3. 16±0.69, respectively (P=0. 000). The result s of ROC diagnosing P53 by FDG SUV demonstrated that the area under curves reached 0. 910±0. 028 (P= 0. 000). The diagnostic critical point of FDG SUVmax in P53 was 3. 785 and the diagnostic sensitivity and specificity were 85.5% and 81.0%, respectively. Conclusion ^18F-FDG SUVmax is significantly correlated with the expression of P53.
