哈萨克族原发性高血压患者血浆视黄醇结合蛋白4水平及其相关性研究 被引量:2

摘要 目的分析新疆哈萨克族(哈族)原发性高血压(EH)患者血浆视黄醇结合蛋白4(RBP4)水平及其与血压、血脂、血糖、胰岛素及胰岛素抵抗指数(HOMA-IR)的关系。方法以2008年8月至11月流行病学调查的新疆哈族人81例为研究对象。哈族EH患者41例为病例组,哈族健康人群40例为对照组。测量血压,测定血浆RBP4、胰岛素、血糖、TC、TG、HDL—C、LDL—C、载脂蛋白A1和载脂蛋白B,计算HOMA-IR。结果①EH组血浆RBP4水平明显高于对照组[(32.74±9.93)mg/L与(21.92±10.43)mg/L,P〈0.01];②EH组血浆RBP4与年龄、收缩压、舒张压、血糖、HOMA—IR呈显著正相关(rs=0.618、0.775、0.565、0.317、0.369,P〈0.01或P〈0.05);在对照组中,RBP4与血糖呈正相关(rs=0.414,P〈0.01)。③多元回归分析显示:EH组收缩压(标准化偏回归系数0.591,t=5.695,P=0.000)进入回归方程,是影响血浆RBP4水平的独立因素。结论哈族EH患者血浆RBP4水平升高,并与收缩压、舒张压、血糖、HOMA-IR呈显著正相关,RBP4可能通过促进胰岛素抵抗而参与了EH的发生、发展。 Objective To explore the plasma level of retinol binding protein 4 (RBP4) in patients with essential hypertension (EH) of Xinjiang Kazakans and the relations between plasma RBP4 level and blood pressure, blood lipids,blood glucose, insulin, and HOMA-IR. Methods 81 Kazakans eases who were investigated between August and November of 2008 were selected as subjects. They were divided into EH group( n = 41 )and control group (n = 40). Blood pressure was measured. Fasting venous blood was drawn to detect the plasma RBP4 ,fasting insulin ( FINS), fasting plasma glucose ( FPG), total cholesterol ( TC ), triglyceride ( TG), high density lipoprotein cholesterol ( HDL-C ) ,low density lipoprotein cholesterol ( LDL-C ), apoprotein-A1 ( Apo-A1 ) and apoprotein-B ( Apo-B ), and homeostasis model assessment of insulin resistance index(HOMA-IR) was calculated. Results ①The plasma RBP4 concentration in EH group was significantly higher than that in control group[ (32.74 ±9.93) mg/L vs (21.92 ± 10.43 )mg/L,P = 0.01 ]. ①RBP4 was positively correlated with age, systolic blood pressure (SBP), diastolic blood pressure (DBP) ,FPG,and HOMA-IR in EH group (rs=0. 618,0. 775,0. 565,0. 317,0. 369,P 〈 0.01 or P 〈 0.05 ), and positively correlated with FPG in control group ( rs = 0. 414, P 〈 O. 01 ). ③Multiple regression analysis showed that SBP was independent factor to affect the RBP4 level in EH group( Standardized Coefficients is 0.591, t = 5. 695,P = 0.000). Conclusion The plasma RBP4 concentration is increased in patients with EH of Kazakans , and is positively correlated with SBP, DBP , FPG, HOMA-IR in EH group. RBP4 may involve in the development of EH by promoting insulin resistance.
