50例儿童系统性红斑狼疮的临床与免疫学特征研究 被引量:1

摘要 目的分析儿童系统性红斑狼疮(SLE)的临床病症与免疫学特征。方法将50例儿童SLE患者(儿童组)与同期的50非儿童SLE患者(成人组)进行对比,分析2组患者发病时的临床症状、体征、系统或器官损害以及血清免疫学指标等。结果儿童组首发症状中出现频率最高的几项依次是:发热、肾脏损害、皮疹、关节痛(或关节炎)和贫血。与成人组相比,儿童组的消化系统损害的发生率32%(16/50)、淋巴结和肝脾肿大的发生率38%(19/50)明显高于成人组消化系统损害及淋巴结和肝脾肿大的发生率14%(7/50)和12%(6/50)(P〈0.05)。在免疫学指标方面,与成人组相比,儿童组的AdsDNA和ARPA的阳性率64%(32/50)和26%(13/50)比成人组42%(21/50)和10%(5/50)明显升高(P均〈0.05)。结论儿童SLE治疗方案的制定需要依据其自身的临床病症与免疫学特征,早期的正确诊断能为SLE患儿争取到最佳的治疗时机,对改善其预后有重要意义。
