神经节苷脂治疗新生儿缺氧缺血性脑病83例临床观察

摘要 目的观察神经节苷脂(GM1)治疗新生儿缺氧缺血性脑病(HIE)的临床疗效。方法将HIE患儿83例随机分为实验组(n=43)和对照组(n=40)。对照组采用常规治疗联合胞二磷胆碱,实验组在对照组基础上加GM1。观察并记录两组患儿的临床表现恢复情况、脑CT变化、NBNA和CDCC评分。结果实验组患儿临床表现恢复情况明显好于对照组,临床总有效率有显著性差异(P<0.05)。两组间轻、中、重度分别相比,仅中度临床有效率有显著性差异(P<0.05)。实验组患儿出生后14d、28dNBNA评分及3个月、6个月CDCC评分均明显高于对照组,有显著性差异(P<0.05或P<0.01)。结论早期联合应用GM1治疗新生儿HIE,能改善神经系统功能障碍,减轻后遗症,值得在新生儿科中推广应用。 Objective To investigate the therapeutic effects of ganglioside(GM1) on neonatal hypoxic-ischemic encephalopathy (HIE). Methods 83 cases of neonates with HIE were divided into control group(n=40) and experimental group (n=43). The patients in control group were given routine treatment combined with citicholine (CDPC). The patients in experimental group were given treatment of GM1 on the basis of control group. Results The recovery of manifestation and changes of brain CT of neonates in experimental group were obviously better than that of neonates in control group(P〈0.05). The NBNA score of neonates in experimental group at postnatal 14 or 28 days increased significantly compared with control group(P〈0.01). The CDCC score of neonates in experimental group at postnatal 3 or 6 months also increased significantly compared with control group(P〈0.05 or P〈0.01). Conclusion GM 1 combined with citicholine on the basis of routine treatment in early stage of HIE can relieve the clinical symptom, reduce brain edema and elevate NBNA and CDCC score of neonates with HIE.
