摘要 目的弄清手工操作免疫凝集测定法(Manual agglutination immunoassay,AIA-M)检验项目的操作工时及其计算方法。方法以工序分解法分解梅毒螺旋体颗粒凝集试验(TPPA)及其他AIA项目的基本工序,用马表计时法测量其工时。结果共分解出38个工序。其中日频工序仅3个,批频工序15个,其余20个为个频工序。所有工序工时的CV值算术平均值为18.24%。工序工时的稳定性与工序本身的规范性及参数的细化程度关系密切。每个项目的批量工时可通过公式计算。对于TPPA的批量工时(mm:ss.0),其计算公式是:(00:43.4)*标本量+(6:10.8)[当n≤12时];其项均工时为03:20.5。由于"固定工时"的存在,项均工时与标本批量成反比关系(γ=[-0.533],P≤0.05)。结论检验项目的基本工序分解、工时测量及其计算方法的确立将使全面弄清同类方法学检验项目的工时成为可能。在以工时量化考核工作量时先考察批量再考察项次量将显得更公平。 Objective To make clear of the operation time(OT) and its calculation equation of the tests with manual agglutination immunoassay(AIA-M). Method To disassemble the elementary operation(EO) of TPPA and other tests with AIA-M with operation decomposition technology, and to measure OT of the EOs with 24-channel stopwatch software. Results Thirty-eight EOs were decomposed from the tests with AIA-M. There were 3, 14 and 20 as the operation per day(OPD), the operation per batch(OPB) and the operation per test(OPT). The arithmetic average of CV of all operation time was calculated to be 18.24%. The stability of the OT was related closely to the standardization of operation itself and the confinement of the operation parameters. The operation time of batch(OTB) of any test could be calculated by a formula. The formula of OTB calculation of TPPA was: OTB(TPPA) =(43:35.3)*n+(06:10:8)[If n≤12]. Its average operation time per test (AOTT) was 03:20.5.The AOTT was inverse relation to the amount of sample per batch( y =[-0.533], P〈0.05). Conclusion That the decomposition of EO of the tests, and that the measurement and establishment of calculation method of OT, made it possible to know the OT of the tests with the same methodology. It would be fair to evaluate quantitatively one's working-burden that the numbers of batch be taken first and then the amount of samples into consideration.
