股骨头缺血性坏死的介入治疗

Interventional treatment of ischemic necrosis of Femoral Head

目的评价介入治疗对股骨头缺血性坏死(ANFH)的治疗效果。方法对489例患者缺血性坏死的股骨头,采用Seldinger穿刺超选择将导管插入ANFH的供血动脉内,后经导管注入罂粟碱,尿激酶,复方丹参,注药后即刻行DSA造影,观察临床症状及股骨头血供改善情况。结果介入治疗后疼痛的缓解或消失占100%,三个月后X线、CT复查均有不同程度的骨质修复,新骨形成。DSA显示股骨细小动脉增多,供血小动脉增粗,静脉显影时间缩短,无并发症,效果满意。结论通过介入方法治疗ANFH是一种安全有效的方法。

Object To evaluate interventional therapy for the treatment of ischemic necrosis of femoral head. Methods 489 patients with ischemic necrosis of the femoral head were enrolled. Using Seldinger technique, the feeding artery was chose and papaverine, urokinase, Dabshen were injected. Results All the patients had pain relief after treatment. X-ray, CT at three months showed bone repair and new bone formation in varying degrees. DSA showed an increase in femoral small arteries, the blood supply in small arterial thickening, vein imaging time. No complications occurred, and results were satisfactory. Conclusions ANFH treatment through the intervention is a safe and effective method.
