精神科住院患者抗菌药物使用的调查

Antibacterial Usage among Inpatients in Department of Psychiatry

目的了解精神科住院患者抗菌药物的使用情况,促进抗菌药物的合理使用。方法对2007年1月1日-12月31日住院的4092例住院患者,在前瞻性监测的同时进行回顾调查分析。结果有471例使用了抗菌药物,抗菌药物的使用率为11.51%,其中治疗性用药例274例(58.17%),预防性用药196例(41.61%)(P<0.001);无指征用药1例(0.21%),单一用药420例(89.17%),二联用药51例(10.83%)(P<0.0001);对治疗用药的病例送检率为0。结论医院在抗菌药物应用指征、药物选择、联合用药、细菌培养送检率等方面存在不合理现象,应加强抗菌药物应用的培训教育、监督管理。

OBJECTIVE To understand the antibacterial medicine usage in inpatients in department of psychiatry to promote the rational use of antibacterial drugs. METHODS Totally 4092 cases of inpatients from Jan 1 to Dec 31 2007 were studied. Both the prospective monitoring and retrospective investigation were carried out. RESULTS From them 471 cases were administrated with antibacterial drugs, and the usage rate was 11.51%, among which the therapeutic administration accounted for 58.17%, and the preventive one of 41.61 % （P〈0. 001）, and one case used drug without any indication（0. 21%）; single medication accounted for 420 cases（89. 17%）, double medication for 51 cases（10.83%）（P〈0. 0001）. The bacterial rate was 0. CONCLUSIONS There exist irrational phenomena in our hospital including indication of antibacterial drugs, medicine selection, combination medication, as well as bacterium culture rate. So training education and inspective management should be enhanced.
